What do I do if I can not find My certification card?

If you have not received or can not find your certification card try these steps;

Check your email for an email with "Rate your program" in the subject line from info@youngheartsed.com
Check your Junk/Spam folder for the email.

If you still can not find your card, please fill out the lost card form.
